This Confidentiality Policy (Policy) is agreed to by all members of the unSCruz Conduct & Consent Committee (CCC) and Santa Cruz Burners LLC. The undersigned agree that involvement in investigations is contingent upon adherence to this Policy.
In order to ensure the safety and privacy of all unSCruz participants involved in a CCC process, and in accordance with CCC policies and applicable laws, it is the policy of the CCC to protect the confidentiality and privacy of those participants who are involved in investigations of reports submitted to the CCC, and to hold confidential all personally identifying information, communications and observations made by, between or about participants, including the identity of the reporter unless one of the Exceptions below applies.
Confidentiality extends to all current and former investigation participants, including those whose complaints were not investigated.
Exceptions of when disclosures of confidential information by a CCC member may occur:
When one of the foregoing Exceptions applies, the disclosure of confidential information shall be limited to the narrow scope of relevant information necessary to coordinate efforts.
Definitions:
“Confidential information” includes any written or spoken information which was shared with the expressed condition that it remain confidential, including any information that might identify the location or identity of someone who has submitted a report.
“Participant” is any person, including the reporter, bystander, witness, subject of a report, unSCruz volunteer, Ranger, Medical Volunteer, CCC member or any other party involved in the investigation.
I have received and read a copy of unSCruz Conduct & Consent Committee Confidentiality Policy and understand that as a CCC member I am bound by this Policy.
